Th350 low /reverse clutches

Pulled apart my th350 with a brake and everything looks good until I get to the low reverse clutches they were welded together. What would have caused this and how do I prevent it from happening again. I also sent my torque converter in to have it checked out and they found the sprag was broken are these too things related. The trans worked fine in reverse but when you shifted from first to second it locked up the tires. Any help would be great. Thanks
 
This could be caused by a number of reasons, including bumping the transbrake button when the car is in motion, a problem with the piston in the back of the case, a crack in the case, a cut lip seal, insufficient clearance in the clutch pack, etc. A welded reverse clutch pack is typically a very uncommon problem with a TH350.
