My last 2 drive shafts I've bought from Inland Empire Drive Line Service that Philip links above. I've never actually had to supply them with measurements, just needed to tell them the car, transmission and rear end. Assuming stock trans location they can figure out the rest. They were dead on with both shafts. One with a TH-350 to a GN 10 bolt and the other with a TH-400 to a 9" inch.

I would shop around locally before you throw down a wad of cash on a driveshaft. I called two big name national company's and their shafts were $550 and $850. They wanted their measurements a certain way. I called a local company that I had done business with many times before. They wanted the measurement taken differently, and they charged $366 for the same 3 1/2 inch chrome moly shaft with the slip yoke and 1350 u-joints.

We have a local guy that makes a great shaft and he guarantees it!, If you break it he will replace it!! and he ships worldwide!

He also has a "how to measure" section

I have one of his drive shafts in my Bu, going on 5 seasons without a problem!
